Requirements:

• Prescriptions will be filled for our current patients only.
• Requests should regard only patients seen in our clinic within the past six months.
• You must be approved by email to our clinic, before you may log on to the pharmacy site.
• Any new symptoms or worsening condition should be reported to us immediately, before requesting any medications online.
